The Borough of Woodbine uses the State's Application for a Certification or a Certified Copy of a Vital Record form. You may use the form for one of the following kinds of records:

  • Birth (except for stillbirths)

  • Marriage

  • Domestic Partnership

  • Death

Download a copy of the form from here. This download includes two forms, 37a (non-genealogical) and 38a (genealogical), which include complete instructions for completing either. Print out and complete the appropriate form and bring it or mail it to the Clerk's office. The form tells you to make a check for the fee.

Pursuant to Chapter 5 of the Code of the Borough of Woodbine, all residents are required to license their dog(s) or cat(s) and provide proof of valid rabies vaccination. Please use this form.
