BOROUGH FORMS

The Borough of Woodbine uses the State's Application for a Certification or a Certified Copy of a Vital Record form. You may use the form for one of the following kinds of records:

• Birth (except for stillbirths)

• Marriage

• Domestic Partnership

• Death

Print out and complete the appropriate form and bring it or mail it to the Clerk's office. The form tells you to make a check for the fee.

Pursuant to Chapter 5 of the Code of the Borough of Woodbine, all residents are required to license their dog(s) or cat(s) and provide proof of valid rabies vaccination. Please use this form.
